Jon Tagatz

Executive Director

Jon Tagatz is the Executive Director of The Heritage at Brentwood, located in Brentwood, Tennessee. He has been employed by Life Care Services since January 1996. He is a licensed nursing home administrator in the States of Tennessee, Florida, Minnesota, and Wisconsin.  Jon received his Bachelor of Science degree in health care administration from the University of Wisconsin – Eau Claire in 1986. He served as the administrator of a city-owned nursing home before joining a non-profit religious based organization in Rochester, Minnesota, where he held the position of administrator for several years. He began his career in life plan community leadership in 1996 when he became the administrator of Sandhill Cove located in Palm City, Florida. He was later named executive director in 1999. He continued his leadership role at Sandhill Cove until 2006 when he transferred to The Heritage at Brentwood. Jon was promoted to Corporate Operations Director in 2014. In this role he provides support to other executive directors within Life Care Services, as well as serving in an advisory role for various construction projects.

Dahlen Jordan

Associate Executive Director

Dahlen Jordan is now Associate Executive Director. In his expanded role he will oversee the community’s Environmental and Residential Health Services departments as well as continuing to serve as the on-site administrator of Somerfield at The Heritage Health Center and Assisted Living. His role is to ensure the highest quality of care and environmental cleanliness for all Residents living in the community. Dahlen holds a Bachelor of Science degree in Business Administration with the emphasis in Management from the University of West Florida in Pensacola, Florida. Dahlen has been an employee of Life Care Services (LCS) since February of 2011, and he serves on LCS’s Health Services team and Operational Council. Prior to joining LCS, Dahlen held the role of licensed nursing home administrator at a variety of Tennessee facilities of varying sizes. In 2002, Dahlen received his Tennessee Nursing Home Administrator License and has been credentialed by the State of Tennessee as a Preceptor. He has successfully trained many other Nursing Home Administrators during his career.

Robert Hagan

Director of Financial Services

Robert has worked with Life Care Services® as a partner and manager of The Heritage at Brentwood® for a combined 11 years. He understands the complexity and details of The Heritage at Brentwood’s Life Plan Community residency agreement, and frequently meets with prospective residents to review their financial questions. Robert is supported by an expert financial team at Life Care Services® to ensure the financial stability and strength of the community.

Mark Weaver

Director of Marketing and Sales

With nearly two decades of experience in the healthcare industry, Mark Weaver has established himself as a seasoned professional in senior living. His journey in healthcare began after obtaining a degree in psychology, which laid the foundation for his patient-centric approach. Mark’s tenure includes 12 valuable years at a Life Plan Community in Bradenton, Florida, where he honed his skills in fostering an environment that allows seniors to thrive.

As the Director of Marketing and Sales at The Heritage at Brentwood®, Mark brings a deep sense of understanding and a commitment to ensuring that residents not only receive exceptional care but also feel empowered to live their best lives. His dedication to patient care is complemented by remarkable patience and a genuine desire to make a positive impact on every individual he interacts with.

Beyond his professional life, Mark is a devoted family man. He and his wife, Alicia, are proud parents to three children—Rio, Cy, and Wren. When not spending quality time with the family, Mark enjoys music and skateboarding.

Cathy Joki

Director of Residential Health Services

Cathy helped open the doors and admitted the first resident to the Somerfield Health Center back in 2007. As the go-to person and health care navigator for independent living residents, she strives to support and coordinate the medical care needed for The Heritage residents. It’s her responsibility to promote each resident’s physical, mental and psychological well-being, thus enhancing their quality of life. Cathy assists residents in navigating through an array of medical services, such as coordinating medical appointments, medication reminders, accessing medical equipment, leading support groups, or working with residents and families to determine appropriate levels of care. Cathy’s goal is to promote an environment supporting the residents’ needs, so they can reach their optimal level of functioning and live as independently as possible.

Jonathan King

Executive Chef

Jonathan joined The Heritage in the fall of 2012. After earning an associate degree in culinary arts at Volunteer State Community College, he completed a three-year apprenticeship at Gaylord Opryland Resort & Convention Center through the American Culinary Federation (ACF). A finalist in LIfe Care Services’ national Chef of the Year competition, Jonathan brings a lifelong passion for food along with new culinary techniques, flavors, and presentation to the dining experience for residents at The Heritage at Brentwood.

Eddie Tomlinson

Director of Facility Services

Eddie joined The Heritage in 2011, where he maintains all the systems that provide a comfortable and safe environment for The Heritage residents. Eddie is also responsible for the maintenance of all residences inside and out, as well as the clubhouses and grounds, so they function flawlessly and are immaculately maintained.

James “Lee” Clark Jr.

Director of Environmental Services

James “Lee” Clark Jr., Director of Environmental Services, comes to The Heritage from Williamson Medical Center in Franklin, where for over seven years he served as Environmental Services Operations Manager. Lee previously served as a manufacturing manager and managed a family owned a construction company. He studied Business Management at University of Phoenix in Franklin. He also studied toward an associate of applied science degree from Columbia State Community College. Lee resides in Columbia with his wife, Dana, and their son Jamie. Hobbies include fishing, bowling and karate, for which he has a 3rd degree black belt. He also has a 2nd degree black belt in Kobudo.

Ashlie Burnett

Director of Community Life Services

Ashlie has been with The Heritage since 2013 and began her Heritage career as the activities director for Somerfield Health Center. In her current role, she is responsible for the coordination of social activities and events identified by residents of The Heritage. Communication is key to a successful and responsive program of multiple activities and topics. With that in mind, a newsletter, in-house TV channels, and monthly calendar form the basis for this communication.

Sarah Selman

Director of Administrative Services

Sarah has worked at The Heritage since mid-2021. She previously worked in operations for the REALTOR® industry for close to 7 years. She supports the executive director and manages the administrative and receptionist services staff. Sarah takes the lead for the business office and administrative functions, as well as serving as the community IT contact.

Mark Black

Director of Food and Beverage Services

Mark joined The Heritage at Brentwood in the winter of 2019. With over 25 years in the hospitality industry, he has served as both executive chef and food and beverage director in hotels, casinos, and resorts. The execution of both high quality service and culinary experience for residents in multiple dining venues at The Heritage at Brentwood requires professional experience and high energy. Mark is dedicated to achieving and exceeding residents’ expectations.

Carroll Bagwell

Director of Human Resources

Carroll has more than 13 years of human resource and management experience. Carroll is responsible for assuring that qualified and responsive staff are recruited and trained for the services they provide residents of The Heritage at Brentwood. Creating a supportive and high-expectation environment for staff, assures that residents’ expectations will be met by staff who enjoy working with residents, as well as having the opportunity for personal fulfillment and growth.
